About the Role
A Nurse Practitioner providing palliative care consultation services in various settings, including nursing homes, hospitals, and community environments.
Responsibilities
- Implement specialist palliative care consultations, including history-taking, examinations, care planning, and communication with patients and families.
- Maintain effective communication with referral sources and ensure accurate, compliant documentation.
- Develop quality initiatives, educate staff, and support hospice operations through participation in on-call rotations.
- Perform clinical and administrative tasks within professional scope, demonstrating professionalism and respect.
- Travel within assigned areas to deliver patient care and coordinate interdisciplinary resources.
Requirements
- Master’s degree in Nursing and graduation from an approved Nurse Practitioner program.
- NY State RN licensure and certification as an Adult, Family, Gerontology, or Palliative Care Nurse Practitioner.
- Preferred: 1+ year clinical experience, hospice or palliative care background, CPR-BLS certification, and hospice & palliative care certification.
- Proficiency in Microsoft Word and Excel.
- Strong communication, listening, organizational, and critical thinking skills.
- Ability to work flexibly, travel within the geographic area, and collaborate effectively within an interdisciplinary team.
