Per Diem Hospice & Palliative Care Nurse Practitioner

About the role

The Nurse Practitioner (NP) provides specialist palliative care consultation services within community, hospital, or nursing home settings, collaborating with a multidisciplinary team to develop and implement individualized care plans.

Responsibilities

• Conduct initial and follow-up visits
• Manage symptoms and facilitate goal-setting discussions
• Review hospice eligibility
• Ensure accurate documentation
• Communicate effectively with referral sources
• Participate in quality initiatives and staff education
• Support hospice operations through on-call duties

Requirements

• Master's degree in Nursing
• NYS RN licensure
• Certification as a Nurse Practitioner (Adult, Family, Gerontology, or Palliative Care)

Qualifications

Preferred experience includes at least one year in hospice, palliative care, or geriatric nursing.
