Per Diem Community Supports Navigator - Adult and/or Children

About the role

The Community Supports Navigator provides support, assistance and instruction to individuals with a developmental disability (ID/DD) and/or autism spectrum disorder (ASD) living independently or at home with a legal guardian. Examples of support include but are not limited to: assistance with apartment maintenance, money management, medical appointments, advocacy, meal preparation and nutrition, socialization, crisis intervention and skill acquisition.

Responsibilities

  • Create and implement Individual Service Plans (ISP), Behavior Plans, Medication Treatment Plans, Assessments, and other relevant documentation within designated time frames.
  • Provide training, modeling, and assistance in activities of daily living and health maintenance to enhance skill acquisition and maintain independent living.
  • Support individuals in developing independence and achieving goals outlined in ISP.
  • Work collaboratively with Ascentria Leadership and Finance to ensure compliance with all rep payee responsibilities, including financial training plan, funds management plan, annual reporting, and bill payment.
  • Transport individuals to and from appointments as needed.
  • Aid in obtaining public benefits such as Social Security, Mass Health, ADA transportation, food stamps, and fuel assistance.
  • Assist individuals when moving between locations, including moving furniture and/or boxes and using personal vehicle to transport client personal belongings.
  • Provide basic problem-solving information and model appropriate social behavior.
  • Serve as an advocate for individuals at various community levels, evaluating the effectiveness of advocacy efforts and making appropriate adjustments.
  • Regularly update and communicate with Program Manager, Clinical Coordinator, and DDS staff.
  • Prepare required paperwork and submit by deadline.
  • Attend and participate in staff, team, and PQI process meetings.

Requirements

  • Bachelor’s Degree in Human Services or related field, or equivalent combination of education and experience.
  • Minimum of 1 year of experience in human services related work, or a minimum of 10 years of experience in human services direct care work with a bachelor’s degree.
  • Licensed or eligible to be licensed by ASWB within 6 months of being hired (LICSW, LCSW, LSW, or LSWA).
  • Demonstrated knowledge of the DDS ISP process and Charting the Life Course.
  • Ability to implement treatment plans and set limits and manage client behaviors.
  • Ability to write accurate and professional documentation in legible English on a daily basis.
  • Ability to set priorities and manage multiple tasks to deadline.
  • Basic Microsoft Office computer skills.
  • Flexible schedule to meet the needs of individuals.
  • Ability to work independently and in a team environment with community-based human service professionals and DDS staff.
  • Valid Driver’s license and proof of automobile insurance is required to transport clients throughout the community.

Qualifications

  • High School Diploma or equivalent with a minimum of 10 years of experience in human services direct care work.
  • Two (2) years of experience in human services required, preferably in community-based settings.
  • Ability to work within the agency’s Mission of strengthening communities by empowering people to respond to life’s challenges.
