Per Diem Community Supports Navigator - Adult and/or Children

About the role

The Community Supports Navigator provides support, assistance and instruction to individuals with a developmental disability (ID/DD) and/or autism spectrum disorder (ASD) living independently or at home with a legal guardian. Examples of support include but are not limited to: assistance with apartment maintenance, money management, medical appointments, advocacy, meal preparation and nutrition, socialization, crisis intervention and skill acquisition.

Responsibilities

  • Create and implement Individual Service Plans (ISP), Behavior Plans, Medication Treatment Plans, Assessments, and any other applicable documentation related to successfully supporting individuals in the community within designated time frames.
  • Provide training, modeling and assistance in activities of daily living and health maintenance in order to enhance skill acquisition and maintain independent living.
  • Work collaboratively with Ascentria Leadership and Finance to ensure compliance with all rep payee responsibilities such as but not limited to: Financial Training Plan, Funds Management Plan, annual reporting, and bill payment.
  • Provide transportation for and escort individuals to and from appointments as needed.
  • Aid in obtaining public benefits such as but not limited to: Social Security, Mass Health, ADA transportation, food stamps and fuel assistance.
  • Aid individuals when moving from one location to another. This may include but is not limited to: moving furniture and/or boxes and using personal vehicle to transport client personal belongings.
  • Provide individual with basic problem solving information. Model and teach appropriate social behavior. Serve as an advocate for individuals at various community levels, evaluate effectiveness of advocacy efforts and make appropriate adjustments.
  • Prepare required paperwork and submit by deadline. Attend and participate in staff, team and PQI process meetings.

Requirements

  • Bachelor’s Degree Required with a minimum of 1 year experience in human services related work
  • HS degree with a minimum of 10 years experience in human services direct care work may be considered as an alternative
  • Licensed or eligible to be licensed by ASWB according to education and experience level (LICSW, LCSW, LSW or LSWA) within 6 months of being hired strongly preferred
  • Minimum of two (2) years of experience in human services required, preferably in community based settings
  • Demonstrated knowledge of the DDS ISP process and Charting the Life Course is strongly preferred
  • Ability to implement treatment plans and set limits and manage client behaviors
  • Ability to write accurate and professional documentation in legible English on a daily basis
  • Ability to set priorities and manage multiple tasks to deadline

Qualifications

  • Valid Driver’s license and proof of automobile insurance is required to transport clients throughout the community

Pay

The pay range is the authorized pay for this position and is displayed only for jobs based in Massachusetts and Vermont, where pay transparency laws require disclosure. Final offers are based on various factors, including skill set, experience, location, qualifications, and other job-related factors.
