Per Diem Community Support Services Counselor

Responsibilities

• Provides IMR and crisis intervention services.
• Affords assistance with food shopping, laundry, and daily living skills.
• Implements prescribed treatment plans and aids in the creation of Individualized Recovery Plans (IRPs).
• Capable of operating independently and traveling to consumers' residences as needed.
• Maintains oversight of medications at consumers' homes.
• Completes required documentation.

Qualifications

• Bachelor of Social Work (BSW) in social work or Bachelor of Arts/Science (BA/BS) in psychology.
• One year of experience in direct care is preferred.
• Valid Driver's License.
