Per Diem Community Support Counselor (CHAMP)

Location: Morris Plains, New Jersey · Employment type: Per diem

Responsibilities

• Responsible for the implementation of IRPs, performing IMR services, and providing continuous input to all changes of consumer IRPs
• Provides on-site case management services to assigned caseload and supervision to consumers
• Facilitates a variety of social, recreational, and therapeutic groups, encourages consumers to participate
• Utilizes crisis intervention skills as needed and reports all consumer concerns to program managers
• Must be able to follow our agency’s documentation procedures correctly when updating our consumers’ electronic charts

Qualifications

• BSW in social work, BS/BA in psychology, or a related field
• Valid Driver’s License

Pay

$19.50 per hour

Schedule

Monday through Sunday, 20 hours per week
