About the role

The Social Worker facilitates care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination. They conduct complex psycho-social assessments and interventions to promote timely throughput, safe discharge and prevent avoidable readmissions.

Responsibilities

• Facilitate care along a continuum through effective resource coordination
• Conduct complex psycho-social assessments and interventions
• Promote timely throughput, safe discharge and prevent avoidable readmissions
• Integrate national standards for case management scope of services
• Ensure compliance with state and federal regulatory requirements
• Provide education to physicians, patients, families and caregivers

Requirements

• Current Clinical Social Work License issued by the Board of Behavioral Sciences
• Master's Degree in Social Work
• Knowledge of and ability to work within a complex health care system
• Excellent written and verbal communication skills in English
• Ability to maintain a work pace appropriate to the workload
• Ability to interpret and implement all applicable standards and regulations such as, The Joint Commission, DHS, and the Child Abuse Registry
• Establish and maintain effective working relationships across the Health System

• Interpret and implement hospital, JCAHO, Health Department and Child Abuse Registry standards and regulations
• Customer service skills appropriate to the job
• Knowledge of University and medical center organizations, policies, procedures and forms
• Ability to establish and maintain effective working relationships across the Health System
