Per Diem and Part time Hospice Registered Nurse

About the Role

We are looking for a dedicated RN to join our Portage-based team, bringing expert clinical care and a warm heart directly to patients and families in the comfort of their own homes.

In this role, you step away from the chaos of hospital corridors to focus on what truly matters: the patient. Your expertise will provide the comfort and peace of mind that allows our clients to live their final days with grace.

Responsibilities

  • Provide personalized, one-on-one clinical care that honors each patient's unique journey
  • Act as a vital pillar of support for families, offering education and emotional reassurance
  • Collaborate with a supportive interdisciplinary team during regular group meetings to refine plans of care
  • Ensure continuity of care through focused supervisory visits and clinical observation
  • Initial and on-going assessment of patient's physical, functional, psychosocial, and environmental needs
  • Implement the individualized plan of care and revisions as needed
  • Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
  • Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, and dealing with issues of ethical concern
  • Supervision of paraprofessionals providing services to patient
  • Preparing clinical and progress notes, participating in IDG conferences
  • Participate in Hospice performance improvement program
  • Maintain high level of customer satisfaction with our clients, families, and caregivers
  • Communication of information using current process and technology within organization
  • Participate in field visits daily and weekly office meetings as indicated
  • Ability to adapt to patient's status and needs on a full-time on-call basis (5 PM - 8 AM)

Requirements

  • Current Michigan RN license in good standing
  • Minimum 1 year of hospice experience
  • Empathetic approach to patients and caregivers
  • Previous experience in a hospice or home health setting
  • Exceptional organizational skills and the ability to work efficiently and independently
  • Flexibility to support our rotating on-call schedule
