Per Assessment - Patient Connect - Nurse Practitioner

About the Role

We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive health assessments within our SNF Patient Connect program. This autonomous role focuses on improving patient outcomes by collaborating with primary care physicians, nursing staff, families, and case management teams.

Responsibilities

• Conduct health assessments to support care coordination
• Collaborate with healthcare providers and families
• Ensure accurate documentation, coding, and gap closure

Requirements

• Active certification (ANCC, AANP) for NPs or NCCPA for PAs
• Unrestricted state licensure
• Driver’s license and reliable transportation
• Ability to work in varied environments and establish collaborative practice agreements if needed

Preferred Qualifications

• Clinical experience, especially in geriatric, long-term care, or home care settings
• Experience with complex behavioral, social, or functional needs
• Proficiency with electronic medical records and technology
