Peer Support Specialist (All Sites)

Durham, Warrenton, Chapel Hill and Roxboro

Responsibilities

• Aid and support consumers during transitions from crisis, inpatient or medical settings to outpatient treatment.
• Collaborate with clinicians, Qualified Professionals (QPs), or discharge planners to develop and implement treatment plans.
• Implement intervention strategies aligned with the treatment plan for consumers and their supports.
• Incorporate consumers into activities designed to enhance various skill sets, manage symptoms, educate on wellness, and prepare for work when applicable.
• Maintain ongoing contact with consumers to ensure smooth transitions between services.
• Arrange transportation for initial appointments.
• Ensure accurate and timely submission of required documentation and assistance with service authorizations as needed.
• Preserve confidentiality and adhere to service protocols.
• Perform other duties and responsibilities as directed by the immediate supervisor.
