About the role
You will be a member of the Mobile Team, assisting the clinicians on Mobile visits and providing support and advocacy to individuals in crisis. You will also be a resource to the clinicians and case manager, through sharing your knowledge of common challenges individuals with SPMI endure and sharing knowledge about community resources.
Responsibilities
- Collaborate with service providers involved with individuals with SPMI.
- Provide information and peer to peer support to individuals who are served by the program.
- Educate and empower clients and their supports regarding their roles in developing crisis plans and in crisis prevention.
- Educate service providers regarding an individual's role in developing a crisis plan for themselves and in crisis prevention.
- Ensure that the individual is an integral part of the crisis planning process.
- Inform service providers (including Mobile Crisis and PCFC providers) regarding person-centered and strength-based practices.
- Maintain and model a focus on strengths.
- Advise Clinical Supervisor.
- Ensure that clients are satisfied with the services they are receiving and, if not, help them advocate for what they need.
- Educate individuals and primary caretakers on how to advocate if services are not meeting their needs.
- Be the liaison to other local supports in the community.
- Link individuals to Peer Support Services activities such as support groups, social events, newsletter.
Requirements
- Requires a High School Diploma or GED.
