About the role
Urban Pathways, founded in 1975, engages New York City's most vulnerable residents, providing essential support and fostering an environment where individuals can redefine their narratives and become their best selves.
Responsibilities
- Provides services and support for clients including assessment, problem-solving, skills modeling, advocacy, training on daily living skills, and money management skills development.
- Conducts field work such as home visits, escorting clients to appointments, and outreach.
- Maintains and submits all necessary documents and reports as required by Urban Pathways, regulatory and funding agency policies, procedures, and regulations.
- Attends staff meetings, professional training, and supervisory sessions, as required.
- Maintains relationships with community services and resources.
Qualifications, Education And Experience
- A high school diploma or GED is required.
- Completion of Peer Advocacy/Specialist Training Program is preferred.
- Experience using behavioral health services (mental health, co-occurring substance use treatment) is required.
- A clean driver license is preferred.
- Veterans and/or experience with veterans are preferred.
- Experience with people experiencing homelessness, mental illness, and/or substance abuse population is preferred.
Skills & Competencies
- Proficiency in Microsoft Office (Word, Excel, Outlook, etc.) and electronic record-keeping systems (Awards, Cares, Caires, etc.).
- Good organizational and time management skills.
- Good communication, interpersonal abilities, and follow-up skills.
- Ability to maintain accurate and up-to-date case notes, documentation, and records.
- Work effectively with individuals from diverse backgrounds and cultures.
- Foreign languages are a plus.
