Key Responsibilities
- Provides supportive assistance with activities of daily living (ADLs) through modeling, direct assistance, and coaching
- Affords clients in obtaining basic needs
- Affords clients an assessment of their emotional well-being, clinical symptomology, behavioral changes, and medication effectiveness
- Offers psychosocial education for consumers regarding mental illness, symptom management, and medication compliance
- Provides crisis intervention as necessary
- Maintains rotating 24-hour on-call coverage as scheduled
- Maintains relationships with state and local hospitals
- Participates in daily treatment team meetings and weekly treatment team planning meetings
Requirements
- High School Diploma or equivalent
- Bachelor's Degree preferred
- One year of experience in the provision of social services for individuals with severe persistent mental illness
- Some lived experience
