Key Responsibilities

• Performing virtual assessment, triage, and care coordination using advanced telehealth technology.
• Providing patient education, supporting family-centered care, and ensuring patient privacy and confidentiality.
• Communicating and collaborating with families, physicians, and interdisciplinary teams to develop care plans.
• Documenting interactions accurately, prioritizing patient needs based on acuity, and participating in quality assurance initiatives.
• Sustaining continuity of care through care plan education and coordination.

Qualifications

• Graduate of an accredited RN program with a BSN.
• Minimum of 5 years of diverse clinical nursing experience.
• Current RN license, with ability to obtain licensure in multiple states if needed.
• BLS certification within 6 months of hire.
