About the role
This position provides face-to-face, virtual, or telephonic care. They collaborate with their team members, both clinical and non-clinical, to coordinate services for patients with chronic or behavioral health/chemical dependency needs across the lifespan. They identify social determinants of health and clinical symptoms requiring intervention, working within the framework of the Interdisciplinary Team (IDT) to build a longitudinal plan of care and meet goals.
Responsibilities
- Coordinate all components of Care Coordination services to address individual patients' health care needs throughout the continuum of care.
- Deliberately organize patient care activities and share information among all relevant participants to ensure safer, more effective care.
- Communicate patients' needs and preferences ahead of time and use this information to provide safe, appropriate, and effective care.
- Follow Renown Health policies and procedures, as well as the Care Coordination Model of Care and Standard Work as defined by CMSA.
- Potentially cross-train within the department to cover for departmental vacations, illnesses, and vacancies.
- Strong interpersonal communication skills, both verbal and written.
- Remains productive and offers help and support to team members.
- Collaborate with the patient, family, providers, and team members to develop a patient-centered Plan of Care and support patient with self-management goals.
- Coordinate alternative community resources such as Home Health Care, REMSA, Durable Medical Equipment, Social Determinants, and Community Partners to promote and assist the patient in having a safe environment aligned with their preferences.
- Facilitate and problem-solve with patients, families, providers, and other healthcare professionals to effectively resolve patient care issues.
- Understand how to navigate the Care Coordination process of Assessment, Planning, Goal Setting, Intervention, and Evaluation, using these components to provide for individual health care needs and promote positive outcomes.
- Help with transitions of care and organize medical information.
- Knowledge of applicable regulatory requirements and community resources.
- Knowledge of continuous quality improvement processes.
Requirements
- Education: Working-level knowledge of the English language, including reading, writing, and speaking English. Appropriate education to obtain and maintain Registered Nursing licensure in the State of Nevada.
- Experience: One year experience as an RN preferred. Care Management or Home Health experience preferred.
- Licenses: Valid State of Nevada or California driver's license and ability to pass Renown Health's Department of Motor Vehicle Report criteria (excluding 200373). Ability to obtain and maintain a State of Nevada RN license at time of hire.
- Background Checks: Fingerprints must be able to pass Nevada Division of Public and Behavioral Health (DBPH) background checks upon hire and every 5 years per State of Nevada Revised Statue (NRS 449.123) to remain in this position.
- Certifications: Utilization or Case Management Certification desirable.
- Computer/Typing Skills: Must possess or be able to obtain within 90 days the computer skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
