Pay Range: LVN $28–$30/hour, RN $32–$35/hour
Responsibilities
- Provide skilled pediatric nursing 1:1 care in patients’ homes
- Perform patient assessments and ongoing evaluations
- Administer medications, treatments, and therapies
- Support hospital‑to‑home transitions for high‑acuity patients
- Educate and support parents, families, and caregivers
- Assist with discharge support and care coordination
- Train and support permanent field nurses during transitions
- Provide first‑shift and interim coverage as needed
- Collaborate with Clinical Care Managers and interdisciplinary teams
- Respond appropriately in emergency situations and changes in patient condition
Qualifications
- Active Texas Registered Nurse or Licensed Vocational Nurse (LVN/LPN) license
- Current CPR certification
- Ability to lift and assist pediatric patients as needed
- Experience with trach care and ventilator management
- High‑acuity pediatric patient experience (preferred)
- Emergency response management experience
- Preferred pediatric home health experience
Schedule & Travel
- Full‑time position with guaranteed hours
- Flexible scheduling based on patient needs and nurse availability
- 5 working days per week, minimum 1 weekend day
- On‑call and staffing coverage responsibilities as needed
- Local travel within approximately 30–40 miles
