About the role
Amazing Care Home Health Services provides high-quality, in-home care to individuals and families. As a Speech Language Pathologist (SLP), you will implement treatment plans to assist clients of all ages with cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning by developing and administering speech therapy services in home and community settings.
You will provide compassionate and high-quality care by performing evaluations, interpreting assessment results, developing a plan of care including skilled interventions and goals, and creating and implementing speech and language treatment plans in conjunction with the physician.
Responsibilities
- Assist pediatric and adult patients to develop or regain cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning to improve their level of independence and quality of life.
- Coordinate care with referring physicians and other healthcare organizations to ensure safe, appropriate, and effective care for the client.
- Instruct clients, families/caregivers, and other healthcare personnel in the client’s treatment plan as indicated.
- Identify client and family/caregiver needs for other services and refer as appropriate.
- Prepare and submit documentation per organization policy.
- Maintain clinical competency in the theory and practice of speech-language pathology.
- Mileage provided for commuting between patient visits.
Requirements
- A master's or doctoral degree in speech-language pathology (SLP).
- ASHA Certification or documentation of passing PRAXIS exam and completion of a Clinical Fellow/Externship.
- Speech-Language Pathologist (SLP) License in the state of Utah.
- Active BLS certification.
- Valid driver’s license and auto insurance.
- A minimum of 1-year clinical experience in Speech Language Pathology is preferred but not required.
Schedule
Part-Time Days, 10-20 visits per week.
