Pediatric Care Manager RN

We are seeking a Care Manager RN to provide comprehensive, patient-centered care management for at-risk patients in an inpatient setting. Responsibilities include assessment, care planning, resource utilization management, discharge planning, documentation, regulatory compliance, and patient advocacy to ensure appropriate healthcare resource use across the continuum.
