Pediatric Care Management Social Worker PRN - Inpatient (Weekends)
About the role
The Pediatric Care Management Social Worker is a PRN position, requiring at least 2 shifts per month. This role is responsible for managing care for high-risk members with chronic behavioral and health conditions, collaborating with members, caregivers, physicians, and the healthcare team.
Responsibilities
- Assesses assigned patients' unique conditions by combining data from medical diagnoses, chart reviews, personal interviews, and care teams.
- Provides social services to patients and their families for an assigned medical service.
- Offers or supervises plan of care interventions to include resource and referral help, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling as appropriate.
- PARTNERS WITH THE HEALTHCARE TEAM AND INVOLVES THE PATIENT AND FAMILY IN THE DEVELOPMENT AND IMPLEMENTATION OF PLANS.
- PROVIDES PATIENTS AND FAMILY EDUCATION REGARDING DISEASES, TREATMENTS, SOCIAL OR FINANCIAL SITUATIONS, EMOTIONAL STRESS AND/OR OTHER PROBLEMS.
- MONITORS CARE TO EVALUATE PROGRESS TOWARDS GOALS AND INSTIGATES APPROPRIATE INTERVENTIONS AS NEEDED.
- CONDUCTS HIGH RISK SCREENING AND PROVIDES CRISIS COUNSELING, EDUCATION AND RESOURCES TO PATIENT AND FAMILIES REGARDING ABUSE OR NEGLECT, PHYSICAL ABUSE, ETC. AND DOCUMENTS THESE SESSIONS.
- ACTS AS A LIASON BETWEEN PATIENTS/FAMILIES AND THE CLINICAL STAFF AND OTHER AGENCIES.
- ADVOCATES FOR PATIENT CONCERNS AND ESCALATES ETHICAL DILEMMA THROUGH THE APPROPRIATE CHANNELS.
- COMPLETES APPLICABLE REPORTS BASED ON MANDATED REPORTING REQUIREMENTS.
- PRECEPTS NEWLY HIRING STAFF MEMBER. PROVIDES FIELD INSTRUCTOR SERVICES TO SCHOOLS OF SOCIAL WORK FOR THE PURPOSE OF TRAINING BACHELOR AND MASTER’S PROGRAM INTERN TO THE HOSPITAL OR HEALTH CARE SETTING.
- DRIVES PERFORMANCE IMPROVEMENT INITIATIVES TO INCLUDE DATA COLLECTION AND RESEARCH, DEVELOPMENT, AND PARTICIPATION IN CONTINUING EDUCATION PROGRAMS.
- MANTAINS AN OPEN, APPROACHABLE MANNER TREATING OTHERS FAIRLY AND RESPECTFULLY PRESERVING SELF-CONFIDENCE AND DIGNITY AS WELL AS SHOW REGARD FOR THEIR OPINION.
- LEAD CARE MANAGEMENT TEAM MEETINGS AND INTERDISCIPLINARY ROUND.
- ASSISTS WITH PROJECTS AS ASSIGNED.
Qualifications
- Master of Social Work from an accredited institution required.
- At least 3 years of social work experience.
- Current Licensed Master Social Worker (LMSW) or under supervision for Licensed Clinical Social Work (LCSW) or LCSW from the Oklahoma State Board of Licensed Social Workers.
- Current Basic Life Support certification from the American Heart Association required.
