Responsibilities
- Supervises daily operations of discharge planning and social services in the inpatient, observation, and emergency department, serving as the primary point of contact for inpatient unit Care Management barriers and concerns.
- Implements corporate Care Management strategies in collaboration with facility management.
- Actively participates in multidisciplinary rounds, ensuring active staff participation and effective discharge planning and barrier resolutions.
- Provides discharge planning and progression of care expertise to the interdisciplinary team.
Requirements
- Knowledge of medical necessity for appropriate patient status and level of care [Required]
- Knowledge of IMM, MOON, Advanced Directives, Multidisciplinary Rounds, Social Services, Value Based Programs, and other Care Management compliance guidelines [Required]
- Education: Bachelor's in Social Work [Required], Master's [Preferred]
- Work Experience: 1+ experience in direct care management [Required], 2+ social work experience [Required]
Qualifications
- Licenses And Certifications: Accredited Case Manager (ACM) [Preferred], Certified Case Manager (CCM) [Preferred]
Skills
- Knowledge of medical necessity for appropriate patient status and level of care
