PD Westchester Community Paramedic | ER at Home & Home Hospital at Home

Job Responsibilities

• Conduct in-person home visits to manage acute illnesses and injuries.
• Provide treatment in line with evidence-based guidelines and collaborate with a multi-disciplinary team.
• Identify risk factors such as medication compliance, limited mobility, impaired nutrition, fall safety, and social isolation.
• Maintain patient confidentiality and complete timely electronic records.
• Adhere to acute care standards.

Vehicle Requirements

• Respond on an on-call basis using a personal vehicle.
• The option to use a Care2U vehicle when available.
• Pick up necessary equipment from designated locations.

Compensation and Job Type

• Per-diem: $40 per hour.
• Inquire about traditional shift opportunities.

• On-call shifts with responses as needed.
• Once monthly scheduled shift to maintain proficiency.

Requirements

• Education: Valid NYS Paramedics Certification.
• Experience: 3 years of experience in 911, Interfacility or Community Paramedicine.
• Certification: BCLS, ACLS, NYS Paramedic.
• Valid NYS drivers license.
• Daily use of PPE and electronic documentation.
• Frequent, prolonged standing, walking, and patient/equipment transfers.
• Ability to carry a 30–40 lb backpack during patient visits.
