About the role

Care2U is a fast-growing healthcare startup providing on-demand, in-home, urgent, and acute care throughout Westchester County, Manhattan, and the surrounding boroughs. We focus on delivering a patient-centered care experience to keep patients in their homes and out of the hospital. This role is ideal for healthcare professionals who value flexible, patient-centered work in Long Island.

Responsibilities

• Conduct in-person home visits to manage acute illnesses and injuries.
• Provide treatment in line with evidence-based guidelines and collaborate with a multi-disciplinary team, including telehealth Emergency Physicians and care coordinators, to handle complex cases.
• Identify risk factors such as medication compliance, limited mobility, impaired nutrition, fall safety, and social isolation.
• Maintain patient confidentiality, complete timely electronic records, and adhere to acute care standards.
• Respond on an on-call basis using a personal vehicle, with the option to use a Care2U vehicle when available. Pick up necessary equipment from designated locations.

Requirements

• Valid NYS Paramedic Certification.
• 3 years of experience in 911, Interfacility, or Community Paramedicine.
• Certifications: BCLS, ACLS, NYS Paramedic.
• Valid NYS driver’s license.

Physical Demands

• Daily use of PPE and electronic documentation.
• Frequent, prolonged standing, walking, and patient/equipment transfers.
• Ability to carry a 30–40 lb backpack during patient visits.

Pay

Per-diem: $40 per hour. Inquire about traditional shift opportunities.

Schedule

• On-call shifts with responses as needed.
• Once monthly scheduled shift to maintain proficiency.
• Attendance at staff meetings.
