Responsibilities

• Facilitate daily unit operations by performing receptionist and clerical duties to coordinate patient care, including initiating discharge folders, patient portal information, and patient information code cards upon admission.
• Assess unit environment and patient rooms daily for needs such as suction, oxygen, and high-risk readmission patients, ensuring nursing staff are aware of compliance steps.
• Coordinate patients’ daily schedules, including tests, exams, procedures, discharge appointments, transportation, and follow-up appointments.
• Maintain and organize patient medical records, perform 24-hour “chart checks,” and ensure orders and consults are correctly entered in the electronic medical record.
• Monitor Service Provider Workspace hourly to communicate stat physician orders and unacknowledged orders to nursing staff.
• Ensure readmission risk patients have Post Hospital Care Clinic follow-up appointments scheduled and documented.
• Receive and record emergent verbal and telephone orders for diagnostic laboratory, non-invasive radiology tests, and nutrition services.
• Proficiently use office equipment such as phones, computers, fax, Vocera, printers, copiers, and scanners.
• Complete requisition forms for clerical, office, dietary, and medical supplies, reconcile orders, and maintain cost-effective supply levels.
• Under the direction of a registered nurse (RN), provide continuous visual and proximal contact with patients, including in-room observation, basic comfort services, maintaining a safe environment, and accompanying patients during transportation.
• Utilize interventions to maintain a safe, therapeutic environment focused on preventing self-harm or injury, and notify clinical staff of any changes in patient status.
• Complete mobility training and stay up to date on all assigned training modules.
• Respond promptly to patient needs, including answering call lights, distributing water, blankets, and pillows, and assisting with feeding and mobility as needed.
• Perform room orientation and instruct patients on communication board usage.
• Provide one-on-one observation for patients who are confused, at risk for injury, or exhibit unpredictable behavior.
• Establish and maintain rapport with physicians, assist with CPOE, troubleshoot issues, and manage consultations and charts.
• Serve as a liaison between physicians and nurses for communication.
• Operate video monitor equipment, manage patient assignments, report changes in patient behavior, and document observations.

Requirements

• BLS certification through the American Heart Association or American Red Cross.
• Familiarity with computers, office equipment, and medical terminology.
• Strong customer service skills and the ability to anticipate and respond to the needs of clinical staff.
• Ability to communicate effectively with diverse individuals and defuse situations to promote a positive patient care environment.
• Demonstrated critical thinking, problem-solving, organizational skills, and responsiveness to crisis situations.
• Able to lift up to 20 lbs and assist with moving patients weighing up to 300 lbs.
• Able to sit or stand, walk, stretch, reach, or bend for 3-5 hours per day.
• Able to sit and focus on patient observation for extended periods, including the entire shift.
• Able to work in a stressful environment and be exposed to patients with infectious diseases.

Preferences

• High School graduate or equivalent, or higher education, or a minimum of 1 year of work experience.
• NAHUC certification desirable.
• Previous experience working in a healthcare or customer service setting preferred.
• Successful completion of a medical terminology course preferred.

• Exposure Category II: Possible, but not routine, potential for exposure to blood, body fluids, or tissues.
