About the Role

Provide each assigned resident with routine daily care and services in accordance with the resident’s assessment and care plan, and as directed by supervisors.

Responsibilities

• Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.
• Report all changes in residents’ condition to the Health and Wellness Director as soon as practical.
• Record all entries on notes, SPAs, and incident reports in an informative, descriptive manner.
• Report all accidents and incidents observed on the shift to the Executive Director and Health and Wellness Director.
• Agree not to disclose residents’ protected health information and promptly report suspected or known violations.
• Report any known or suspected unauthorized attempt to access the community’s information system.
• Perform only those care tasks for which you have been trained.
• Prepare residents’ rooms (e.g., make beds) and greet/escort residents to their rooms.
• Introduce residents to roommates and other personnel as appropriate.
• Inventory and mark residents’ personal possessions; store clothing and assist with packing/unpacking during transfers or discharges.
• Transport residents to new rooms or receiving areas; assist with loading/unloading residents to/from vehicles.
• Maintain a warm, calm, and positive environment throughout the unit and shift.
• Meet regularly with nursing personnel to identify and correct problem areas or improve services.
• Report all resident complaints and grievances.
• Participate in and receive nursing reports as instructed.
• Follow established policies concerning exposure to blood/body fluids.
• Perform daily care tasks, including dental/mouth care, bathing, dressing, hair care, nail care, and shaving.
• Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
• Assist residents with meals (e.g., identifying food arrangements, post-meal care).
• Keep water pitchers clean, filled, and within easy reach of residents.
• Assist with lifting, turning, moving, positioning, and transporting residents (e.g., in/out of beds, chairs, bathtubs, wheelchairs).
• Perform restorative and rehabilitative procedures as instructed.
• Observe and report pressure areas, skin breakdowns, and injuries of unknown source.
• Monitor disoriented and comatose residents; record and report data as instructed.
• Perform special treatments and assist with physical examinations as instructed.
• Immediately notify the Nurse of any resident leaving or missing from the facility.
• Use only equipment you have been trained to use; report defective equipment.
• Maintain confidentiality of resident care information, including protected health information.
• Report allegations of resident abuse and/or misappropriation of resident property.
• Honor residents’ refusal of treatment and report such requests to supervisors.
• Assist with bowel and bladder functions (e.g., bathroom, bed pan, urinal, portable commode).
• Maintain intake and output records as instructed.
• Provide eye and ear care (e.g., cleaning eyeglasses/hearing aids, installing hearing aids).
• Weigh and measure residents; record temperatures, pulse, and respirations (TPRs) as instructed.
• Check residents routinely to ensure personal care needs are met.
• Assist with end-of-life care and post-mortem care as instructed.

Requirements

• High school education or equivalent (minimum).
• Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Proficiency in applying mathematical concepts (fractions, percentages, ratios, proportions) to practical situations.
• Ability to solve practical problems with limited standardization and interpret instructions in written, oral, diagram, or schedule form.
• Proficiency in Microsoft Suite products.

Physical Demands

• Occasionally lift and/or move up to 25 pounds.
• Prolonged use of desktop or laptop computers.
• Regularly required to sit, stand, walk, talk, read, or hear.
• Frequent use of office-related equipment (copier, scanner, fax, telephone, calculator).

Work Environment

The noise level in the work environment is usually low to moderate.

Pay

Starting at $19/hour, depending on experience.
