Patient Transport Driver - LYN Pace Transport/PRN Days

About the role

This position provides transportation services for Centra PACE/Ground Transport under the direction of the Transport Supervisor(s). The Driver ensures a safe and enriching experience for PACE participants and patients during door-to-door transport to and from Centra facilities.

Responsibilities

• Provide exceptional door-to-door care to all PACE participants and Centra patients.
• Communicate participant/patient changes and needs to the direct supervisor.
• Perform pre- and post-vehicle safety inspections, ensure operational condition, and report maintenance issues.
• Maintain cleanliness of the interior and exterior of Centra vehicles.
• Follow scheduled transports for PACE participants or patients and other assigned trips.
• Maintain all necessary records and documentation regarding participants, patients, and vehicles.
• Enter participants’ homes safely on a routine basis.
• Participate as a member of the interdisciplinary team in care plans under the Transport Supervisor’s direction.
• Meet standardized competencies for the position as established by Centra PACE and regulatory agencies.
• Deliver participant medications, medical supplies, and other necessities.
• Perform other duties as assigned.

Requirements

• Valid driver's license in the United States.

Pay

$15.90 - $22.26 per hour.
