Patient Transfer Coordinator (CMA, EMT, LPN) - 17794

Qualifications

• High school diploma or equivalent required
• Clinical Medical Assistant, EMT, A-EMT, EMT-P, or LPN required
• If Clinical Medical Assistant, completion of a Medical Assistant Program with eligibility to sit for NHA, NCCT or AAMA required
• Typing and computer skills
• Able to prioritize and multi-task duties
• Able to effectively communicate with coworkers, physicians, patients and administration
• Medical terminology background
• Strong interpersonal skills
• Computer literacy and data entry skills

Responsibilities

• Receives and coordinates calls regarding acute patient transfers, consults and/or admission from medical facilities
• Documents the transfers and consults from other medical facilities into transfer center program
• Utilizes patient logistics software to coordinate patient flow and bed placement
• Speaks directly to the referring and receiving provider/staff to coordinate patient care
• Coordinates the hospital rotation for Penn Highlands DuBois
• Takes calls for emergent and non-emergent transportation
• Schedules and arranges transport via van, ambulance or helicopter (depending on needs)
• Documents all transports as appropriate
