About the Role

Support the connection between patients, families, and the compassionate care they need. As a Hospice and Physician Services Intake Coordinator, you'll serve as a critical first point of contact by managing referrals, coordinating information, and ensuring patients are connected with the appropriate services in a timely manner. From answering inquiries and entering referral information to partnering with clinical teams and referral sources, you'll help create a seamless experience that supports both exceptional customer service and quality patient care.

This role is ideal for someone who thrives in a fast-paced healthcare environment, enjoys helping others, and takes pride in organization and accuracy. You'll manage the referral process from intake through transition to care, maintain communication with supervisors, sales teams, providers, and community partners, and ensure accurate documentation within company systems. Through your attention to detail, responsiveness, and commitment to service excellence, you'll play an essential role in supporting the growth of Hospice and Physician Services while helping patients and families access the right care at the right time.

Location

Office Location: Jackson, MI

Schedule

On-Site Monday to Friday, 10 a.m. to 7 p.m.
Remote Weekend Rotation Required

Qualifications

  • High school diploma or GED
  • Reliable transportation, valid driver's license, and valid automobile insurance coverage

Preferred Qualifications

  • Associate's degree
  • Minimum 1 year of experience in a healthcare setting
  • Knowledge of Medicare, Medicaid, and third-party payor guidelines preferred

Skills

  • Interpersonal skills and ability to communicate professionally and effectively
  • Knowledge of medical terminology
  • Strong verbal and written communication skills
  • Ability to operate computer and office equipment
  • Proficiency in Microsoft Office Suite and data entry skills

Pay

Pay Range $19—$22 USD
