Patient Service Line Navigator - Acute Care Surgery
Patient Care
Provides education, acts as a liaison with the patient, physician, family, other health care personnel, hospital administration, agencies and community groups throughout the patient's hospital stay or service.
Serves as point of contact, clinical leader and resource to staff and administration.
Supports and guides patient and family in the health care decision-making process including but not limited to admissions, diagnostics, surgical/medical interventions, discharge, and support services.
Provides patients and family with needed information for the transition from active treatment to managing their condition independently.
Technical Skills and Responsibilities
- Collaborates with multiple service lines to provide multi-system focused patient care for trauma and emergency general surgery patients.
- Requires strong teamwork, communication, and organization skills to serve a complex patient population.
- Directly supports Trauma and Emergency General Surgery Programs under the Acute Care Surgery Institute.
Qualifications
- Bachelor's in Nursing (BSN) from an accredited School of Nursing (or must obtain BSN within 2 years of hire), with licensure to practice as a Registered Nurse by the State of Florida.
- OR a Master's of Social Work (MSW) degree from a Council on Social Work Education (CSWE) accredited college or Social Work program.
- OR a Master’s degree in Health Administration or related field.
- Three years related work experience required.
