Responsibilities

• Ensure patient assessment visits, including all Oasis visits, are scheduled and performed timely.
• Assign and schedule clinicians efficiently by geography, matching skills with required care and optimizing capacity.
• Collaborate with field clinicians to ensure all patient visits are scheduled and completed as ordered within the care center.
• Manage calls from patients and field staff related to scheduling issues.
• Refer clinical and performance-related issues to the clinical manager.
• Monitor hospitalized patients and ensure follow-up by the team.
• Support the clinical manager and collaborate to ensure timely communication with patients, clinicians, referral sources, and office personnel.
• Run applicable reports and respond to workflow needs with appropriate actions.
• Assist with internal or external transfer of patients between care centers and/or hospice services.
• Maintain supply closet with routine supplies, ensuring items are within expiration dates and properly packaged; serve as liaison for patient supply needs.
• Perform other duties as assigned.

Qualifications

• High School diploma or GED equivalent.
• Preferred: One year of scheduling experience in a healthcare environment.
• Preferred: One year of administrative experience in a healthcare environment.
