Responsibilities
- Ensures patient assessment visits, including all Oasis visits, are scheduled and performed timely
- Assigns and schedules clinicians efficiently by geography, maximizing clinician efficiency, matching skills with required care, and optimizing capacity
- Collaborates with field clinicians to ensure all patient visits are scheduled and completed as ordered within the care center
- Manages calls from patients and field staff related to scheduling issues
- Refers clinical and performance-related issues to the clinical manager
- Monitors hospitalized patients and ensures follow-up by the team
- Supports the clinical manager to ensure timely communication with patients, clinicians, referral sources, business development team, and other office personnel
- Runs applicable reports and responds to workflow, taking appropriate actions
- Assists with internal or external transfer of patients between care centers and/or hospice services
- Maintains supply closet with routine supplies, ensuring items are within expiration dates and packaged appropriately, and serves as liaison for patient supply needs
- Performs other duties as assigned
Requirements
- High School diploma or GED equivalent
- Preferred: One year of scheduling experience in a healthcare environment
- Preferred: One year of administrative experience in a healthcare environment
