Responsibilities
- Schedules home visits for field staff and assists with coordination of patient care under the direction of the Clinical Manager.
- Ensures patient assessment visits, including all Oasis visits, are scheduled and performed timely.
- Prepares weekly clinician schedules and reviews them daily to determine capacity opportunities and ensure productivity standards are met.
- Assigns and schedules clinicians in the most efficient geography, maximizing efficiency, matching skills with required care, and optimizing capacity.
- Ensures missed visits are scheduled by the end of the current week or referred to the Clinical Manager for resolution.
- Manages calls from patients and field staff related to scheduling issues; refers clinical and performance-related issues to the Clinical Manager.
- Monitors hospitalized patients and ensures follow-up as necessary.
- Completes requested schedules, ensuring all orders and services (e.g., lab draws, supervisory visits, add-on visits) are included.
- Supports the Clinical Manager and collaborates to ensure timely communication with patients, clinicians, referral sources, and other personnel.
- Runs applicable reports and responds to workflow, taking appropriate actions.
- Assists with internal or external transfer of patients between care centers and/or Hospice services.
- If licensed and qualified, may provide direct patient care in emergencies and maintain the supply closet, ensuring supplies are within expiration dates.
- In the absence of an Intake Coordinator, may handle the referral and intake process within the care center.
- If a licensed clinician, may obtain verbal orders for patients and document them as applicable.
- Performs other duties as assigned.
Requirements
- Current, unencumbered LPN/LVN or RN license specific to the state of employment.
- High School diploma or GED equivalent.
- One (1+) year of administrative experience in a healthcare environment.
Qualifications
Preferred: One (1+) year of referral and scheduling experience in a healthcare environment.
