Responsibilities
- Serves as patient safety expert consultant, resource, and collaborator for clinical leadership/staff, medication safety officer, medical safety coordinator, and related safety committees.
- Maintains occurrence notification system, root cause analysis, and action plans as indicated.
- Manages the occurrence notification system by identifying trends and issues related to patient safety and clinical outcomes and referring reports for follow-up and action planning.
- Manages and conducts investigation of events and/or root cause analysis of events as assigned.
- Identifies action plans to improve patient safety based on risks identified through investigations and works collaboratively to implement error-reduction strategies.
- Serves as safety coach mentor for clinical departments and promotes effective internal communication on issues of patient safety.
- Develops and provides education for physicians and clinical staff relative to patient safety issues and trends identified.
- Ensures system compliance with Joint Commission National Patient Safety Goals and other regulatory initiatives.
- Conducts patient safety rounds on a regular basis.
- Analyzes the human, organizational, process, and systems factors that contribute to adverse events in collaboration with performance improvement consultants.
- Maintains professional development by expanding knowledge base of methodologies/techniques, facilitation/interviewing/investigation skills, and the use of continuous quality improvement tools.
- Represents Patient Safety on system level committees and workgroups as assigned providing their input on safety risks and mitigation strategies.
- Functions as subject-matter expert and program owner for defined ONS “buckets” and safety domains.
- Manages ONS intake, triage, trend identification, escalation, and event closure.
- Reviews ONS and related data for trends that require action and oversees serious event management processes and outcomes.
- Facilitates complex RCAs for serious events and near misses.
- Manages and conducts investigation of events and/or root cause analysis of events.
- Conducts in-depth chart reviews, policy reviews, staff interviews, and workflow mapping; partners with providers to discuss medical decision making.
- Partners with risk managers to ensure thorough, efficient event evaluation and a coordinated institutional response.
- Partners with accreditation to plan, support, and evaluate events with a regulatory component, ensuring requirements are met and learnings are shared related to safety practices.
- Partners with leaders to develop action plans, implement, and monitor action plans resulting from event investigations and RCAs.
Qualifications
- 5 years of RN experience in a clinical setting; pediatrics preferred
- Previous experience in general pediatrics, PICU
- Experience with MS Word, Excel, PowerPoint
- Knowledge of Joint Commission and other applicable regulatory standards
- Experience in process improvement or related field
- Certified Professional in Patient Safety (CPPS) through the Institute for Healthcare Improvement
- Bachelor of Science in nursing (preferred), or graduation from an accredited school of nursing and a baccalaureate degree in another field, or graduation from an accredited school of nursing and 5 years of progressing nursing experience
- Certification Summary: Licensure as a Registered Nurse in the single State of Georgia or Multi-State through the Enhanced Nurse Licensure Compact
