Schedule

  • Monday through Friday from 8:30 AM Eastern Standard Time to 5:00 PM Eastern Standard Time
  • Work schedule is subject to change based on business needs

  • Hybrid role based in Syracuse, NY
  • On-site three (3) days per week and remote two (2) days per week after successful completion of on-site training

About the role

The Patient Referral Coordinator is responsible for the processing and coordination of all new referrals with the main objective being to clear all patients prior to service in order to ensure proper payment in compliance within all legal and regulatory parameters. This role ensures accurate, efficient, and timely processing of new referrals for Infusion and Enteral services, including obtaining necessary documentation, confirming insurance coverage, coding and pricing products, documenting patient information, submitting for pre-authorization, and coordinating the initial referral push. The Admissions Representative provides support and collaborates with referral sources, hospital liaisons, pharmacy, nursing, billing, and branch personnel.

Responsibilities

  • Provide knowledge and leadership regarding infusion intake processes by:
    • Receiving new requests for services via phone, fax, or other means
    • Obtaining all information necessary to accurately process the referral according to established procedures
    • Completing the referral process within the required timeframe to meet patient, family, and referral source needs
    • Accurately completing the patient record in the applicable computer system, including demographics, insurance, and clinical information
    • Recording progress notes and alerts to present a clear picture of patient data
    • Confirming insurance coverage for requested services, including eligibility, deductibles, out-of-pocket costs, authorization requirements, and patient obligations
    • Communicating with referral sources or liaisons regarding insurance coverage, information limitations, and referral status
    • Obtaining insurance authorization when necessary, documenting it according to procedure, and tracking expiring authorizations for renewal
    • Ensuring accurate and complete billing information is obtained and recorded for timely claim submission and payment
    • Communicating financial obligations to patients or their designees, ensuring understanding and agreement, and establishing payment methods up-front
    • Maintaining the pending referral log daily and attending morning rounds prepared to discuss patient status
    • Responding to requests for assistance from pharmacy, billing, and clinical personnel
    • Staying familiar with specific payer guidelines, requirements, and contracts
    • Staying current with and implementing industry changes
    • Consulting with the Admissions Director or Manager for unfamiliar situations or decisions outside the role’s scope
    • Assisting with special projects and performing other duties as needed
    • Maintaining the monthly Referral Census Log
    • Serving as a company resource for Infusion processing and reimbursement
    • Demonstrating a professional, cooperative, and friendly attitude toward all internal and external customers
    • Using discretion in handling confidential matters

Requirements

  • Associate’s degree in Business, Health Care Services, or Medical Sales
  • Minimum of one (1) year of experience in the healthcare industry
  • Strong working knowledge of first- and third-party payer requirements related to billing and collections
  • Excellent management, communication, interpersonal, teamwork, organizational, and analytical skills
  • Ability to prioritize and multitask
  • Proficiency in problem resolution
  • Self-motivated and self-disciplined
  • Computer and mathematical aptitude

Qualifications (Preferred)

  • Minimum of two (2) years of experience with a home care company
  • Prior home infusion experience on the provider or payer side
