About the role

As a Patient Outreach Specialist at Quincy Medical Group, you will play a vital role in expanding access to primary care by proactively connecting with patients who are due for care. This dedicated outreach position focuses exclusively on contacting patients who have not reestablished care, are overdue for annual wellness visits, or require follow-up appointments for medication management and chronic conditions.

You will work closely with primary care clinics, helping to reconnect patients with their providers, improve continuity of care, and support the long-term health of the communities we serve. This position is ideal for someone who enjoys building relationships over the phone, is motivated by measurable results, and is passionate about helping patients take the next step in their healthcare journey.

Responsibilities

  • Conduct outbound calls to patients who have not reestablished care, are overdue for annual wellness visits, or require follow-up appointments.
  • Schedule new and returning patient appointments while helping patients find the provider and appointment time that best meets their needs.
  • Educate patients on the importance of preventive care, chronic disease management, and maintaining an ongoing relationship with a primary care provider.
  • Maintain accurate documentation of all patient outreach activities within the electronic medical record.
  • Track outreach efforts, scheduling outcomes, and patient engagement metrics.
  • Cook up with clinic staff to resolve scheduling barriers and improve patient access.
  • Identify opportunities to improve outreach processes and patient conversion rates.
  • Provide exceptional customer service while building trust and rapport with patients over the phone.
  • Maintain confidentiality and comply with HIPAA and organizational policies.
  • Perform other duties as assigned to support patient access and organizational growth initiatives.

Qualifications

  • A high school diploma or GED is required.
  • Previous experience in healthcare scheduling, patient access, medical office administration, customer service, call center operations, or a related field is preferred.
  • Strong communication and interpersonal skills with the ability to build rapport over the phone.
  • Excellent organizational skills and attention to detail.
  • Ability to work independently while managing daily outreach goals.
  • Comfort using multiple computer systems and electronic medical records.
  • Strong problem-solving skills and ability to navigate scheduling challenges.
  • Self-motivated with the ability to manage priorities and achieve measurable outcomes.
  • Commitment to providing an exceptional patient experience.

Pay

The compensation for this role includes a base pay range of $16.12 - $24.18, with actual pay determined by experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through bonuses and other incentives.

Base pay is only a portion of the total rewards package.
