Job Summary

The Patient Navigator plays a vital role in supporting patients throughout their healthcare journey by providing individualized assistance to help them access, understand, and utilize healthcare services effectively. This position focuses on reducing barriers to care by coordinating appointments, facilitating communication between patients and providers, and connecting patients to community resources and support services. The Patient Navigator works closely with clinical teams to ensure timely follow-up, promote patient engagement, and improve health outcomes, particularly for those managing complex or chronic conditions.

About the Role

This role requires strong interpersonal skills, excellent organizational abilities, and the ability to work independently and as part of a team. The Patient Navigator must be able to manage multiple tasks simultaneously and prioritize responsibilities effectively.

Responsibilities

  • Coordinate patient appointments and communicate with providers to ensure timely follow-up
  • Facilitate communication between patients and healthcare providers to address concerns and questions
  • Connect patients to community resources and support services that can enhance their healthcare experience
  • Work closely with clinical teams to ensure comprehensive care plans are implemented
  • Provide educational materials and resources to help patients better understand their healthcare options
  • Monitor patient progress and report any issues to appropriate parties

Requirements

  • Minimum Qualifications: Bachelor's degree
  • One-year relevant experience
  • An equivalent combination of education and experience relevant to the role may be considered for this position

Qualifications

  • Required: May require licensure or certification specific to the service line the position is assigned to

Skills

  • Strong interpersonal and communication skills
