Patient Navigator, Surgery (RN)- ENT - Physician Practice
About the role
As a Patient Navigator, Surgery at Hackensack Meridian Health, you will serve as an advocate and educator for patients and their families throughout the surgery/cancer continuum. You will facilitate communication between patients and healthcare providers, provide education and resources, and coordinate care to ensure timely and appropriate treatment.
Responsibilities
- Treat all patients with dignity and professionalism while providing competent clinical care during office visits.
- Serve as a content expert in the specialty practice, acting as a strong patient advocate.
- Facilitate communication between patients and healthcare providers, providing education and resources to patients and families.
- Greet patients courteously, efficiently process their information, and complete physician’s orders accurately.
- Guide patients through the healthcare system, addressing psychological aspects of cancer and coordinating appointments for diagnostic and therapeutic services.
- Answer patient telephone calls, triage urgent/emergent calls, and assist with scheduling, problem-solving, and general inquiries.
- Communicate professionally with physicians, relay messages, and ensure all necessary materials (films, pathology reports, lab results) are available for appointments.
- Advise patients on required records and forms for visits, prepare new patient records, and elicit clinical information for appropriate referrals.
- Encourage and schedule referrals based on clinical presentation, following established protocols or physician directives.
- Identify and address barriers to care, connecting patients with resources, social support, and financial services.
- Collaborate with clinical research teams to identify potential candidates for clinical trials.
- Network with hospital and community services, provide community education, and participate in practice hours across locations as needed.
Requirements
- RN (Associate’s, Diploma, or BSN) with a current NJ State Professional Registered Nurse License.
- Minimum of 3 years of nursing experience in a related field (Surgery/Oncology).
- Strong computer skills and excellent written/verbal communication abilities.
- Proven organizational and critical thinking skills.
Pay
The minimum base pay for this role is $113,609.60 annually. The starting rate may vary based on factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, and budget considerations. Eligible positions may also include shift differentials, performance-based incentives, bonuses, or commissions.
