About the role
The Patient Navigator Program at NewYork-Presbyterian Queens empowers and educates patients on accessing and utilizing healthcare services. The role supports patients and caregivers in creating effective care plans and connecting them with the right health and social services.
Responsibilities
- Care Coordination: Scheduling and managing patient appointments, providing logistical support, and facilitating communication between patients and the medical team.
- Support: Addressing patient inquiries, directing them to the appropriate staff, and assisting with the collection of medical records, insurance details, and financial information.
- Communication: Engaging with patients in person, over the phone, and via email to ensure seamless coordination of care.
Requirements
- Bachelor's degree
- 2-3 years of experience in primary or community health
- Familiarity with hospital operations and community resources
- Fluency in Mandarin, Cantonese, or Spanish (preferred)
Preferred Criteria
- Previous experience in healthcare
- Understanding of medical office practices and procedures
- Strong organizational, multi-tasking, and time management skills
- Flexibility, persistence, and patience in a dynamic environment
- Excellent oral, written communication, and interpersonal skills
- Proficiency in Microsoft Word and Excel
