Patient Navigator (RN)- Preoperative

About the role

The Registered Nurse – Patient Navigator – PREP role is a critical member of the preoperative care team, working in close partnership with a small group of APPs and physicians who manage the assessment and optimization of medically and socially complex patients.

Responsibilities

• Understand the clinical pathways established by the PREP program and provide proactive clinical support to coordinate care.
• Close communication gaps and manage care gaps to promote timely preoperative optimization.
• Promote patient outcomes through coordination of care and support for medically and socially complex patients.

Requirements

• Experience working with preoperative patients (ASU) and families preferred.
• Experience in community outreach preferred.

Qualifications

• Bachelor’s of Science in Nursing (BSN) degree required.
• Master’s Degree preferred.
• Current License to practice as a Registered Professional Nurse in the State of Maine.
• Evidence of ongoing education to maintain appropriate licensing.

Schedule

Full Time/Days (40 hours per week).
