About the role
The Registered Nurse – Patient Navigator – PREP role is a critical member of the preoperative care team, working in close partnership with a small group of APPs and physicians who manage the assessment and optimization of medically and socially complex patients.
Responsibilities
- Understand the clinical pathways established by the PREP program and provide proactive clinical support to coordinate care.
- Close communication gaps and manage care gaps to promote timely preoperative optimization.
- Promote patient outcomes through coordination of care and support for medically and socially complex patients.
Requirements
- Experience working with preoperative patients (ASU) and families preferred.
- Experience in community outreach preferred.
Qualifications
- Bachelor’s of Science in Nursing (BSN) degree required.
- Master’s Degree preferred.
- Current License to practice as a Registered Professional Nurse in the State of Maine.
- Evidence of ongoing education to maintain appropriate licensing.

Schedule
Full Time/Days (40 hours per week).
