Core Job Functions

  • Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.
  • Ensures patients have access to insurance and social service programs for which they are eligible.
  • Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.
  • Identifies and resolves real or perceived barriers to care.
  • Educates and counsels patients on issues related to their health and life situations.
  • Maintains current knowledge of community health, welfare resources, and any other programs available to patients.
  • Handles patient communications and contacts healthcare facilities for medical records.
  • Addresses care gaps with the nurse navigator.
  • Prepares, tracks, and processes medical records and pathology specimens.
  • Reviews and files medical records for completeness; uses tracking systems for accurate documentation.
  • Provides administrative support to the department by answering phones, addressing inquiries, and directing calls or taking messages.
  • Copies, faxes, emails, and mails records while ensuring confidentiality.

Core Qualifications

  • Education: High School diploma or equivalent required.
  • Experience: Minimum 1 Year Of Relevant Experience Required.
  • Certification and Licensing: Refer To Department Description For Applicable Certification Requirements.
