Medical Navigator - Per Diem

Job Responsibilities

  • Process and transition clients through admissions including completion of all required documentation.
  • Meet with client’s family obtaining information related to client’s history as it pertains to admission.
  • Cookordination with all admission related staff ensuring expediency of client admission including Utilization Review, Nursing, Admission Specialists and staff in related departments.
  • Obtain and observe urine drug screens, take client vitals, and report abnormal values/trends to staff nurse.
  • Complete safety checks throughout the clients stay.
  • Orient clients to the rules and regulations for clients at Rushford.
  • Provide basic counseling to help decrease the client anxiety and assure continuity of care.
  • Manage clients’ discharge by ensuring all property is returned, forms are signed and documentation is complete.
  • Run non-clinical groups.
  • Maintain supplies by managing inventory and ordering as needed.
  • Complete audits as needed.
  • Serves as a liaison between Rushford and outside contracting services, providing prompt service for all requests.

Qualifications

  • A High School graduate or equivalent.
  • Diploma/Certificate from an accredited Medical Assistant program, OR an active CT CNA or EMT Certification, OR relative experience as outlined below.
  • Diploma from an accredited Medical Assistant program or Associate’s Degree in related field of substance abuse and/or mental health preferred.
  • In lieu of education requirement, at least one year of direct medical patient care may be considered.
  • Previous experience in drug & alcohol treatment or disease concept of addiction preferred.
  • Previous experience with mental health and/or psychiatric population preferred.
  • Familiarity with Alcoholic (or Narcotics) Anonymous 12-Step Program preferred.
  • Obtain CPR certification within six months of employment. AED Training within six months of employment.
  • All required credentials to be maintained throughout employment.
  • Average of one year of keyboarding and computer skills with working knowledge of Microsoft Office products.
  • Demonstrated organizational and multi-tasking skills.
  • Good interpersonal/written communication skills.
  • Ability to perform duties with minimal direction.
  • Customer Service and detail oriented.
  • Maintain confidentiality and professionalism at all times.
  • Able to function as a member of a team.
  • Model quality client/customer services by: demonstrating a willingness to serve all customers at all times; maintain an awareness of patient needs and utilize the initiative to respond to those needs within skill level; utilize tact, discretion, and compassion in dealing with other members of the team to ensure quality patient care.
  • Demonstrate willingness for self-improvement by collaborating with leadership to identify learning needs (reflective of the organization and of the individual) and seek instruction from appropriate resources.
  • Direct patient care experience” be defined as medical direct patient care experience---this type of experience includes vital signs, point-of-care testing, phlebotomy, lab processing, facilitating patient flow, etc.
