About the role
The Wraparound Hospital-based Violence Intervention Program is under the UC Davis Medical Center, Patient Care Services Department. The role supports Wraparound patients' post-injury recovery and facilitates their transition from trauma center services to community care.
Responsibilities
- Support healthcare system and post-injury recovery community service patient navigation (80%)
- Provide Wraparound Program Support (10%)
- Document patient care interventions (10%)
Requirements
- Completion of CITI Basic course for Social and Behavioral Researchers and Staff within 3 months of hire.
- Completion of Mental Health First Aid training within 6 months of hire.
- Completion of Violence Prevention Professional Certification within 1 year of hire.
- Bachelor's degree in related area or equivalent experience/training.
- Experience working with youth and young adults affected by violence or involved with the Justice System.
- Experience working in a Level 1 Trauma Center Hospital-based Violence Intervention Program.
- Strong interpersonal and customer service skills.
- Ability to communicate and resolve issues effectively with a diverse population of patients, staff, and physicians.
- Excellent analytical and problem-solving skills.
- Ability to develop solutions and recommend changes and follow through with implementation.
- Excellent written and verbal communication skills.
- Proficiency with Windows-based software including Microsoft word, Excel, Outlook.
- Knowledge of computer systems and software used in functional area.
Qualifications
- Experience connecting individuals to needed social services in Sacramento County.
- Experience providing mentorship for youth affected by violence.
- Personal experience in overcoming violence and violence-related injuries.
- Knowledge of intervention techniques and trauma-informed practices.
