About the role

The Wraparound Hospital-based Violence Intervention Program is under the UC Davis Medical Center, Patient Care Services Department. The role supports Wraparound patients' post-injury recovery and facilitates their transition from trauma center services to community care.

Responsibilities

  • Support healthcare system and post-injury recovery community service patient navigation (80%)
  • Provide Wraparound Program Support (10%)
  • Document patient care interventions (10%)

Requirements

  • Completion of CITI Basic course for Social and Behavioral Researchers and Staff within 3 months of hire.
  • Completion of Mental Health First Aid training within 6 months of hire.
  • Completion of Violence Prevention Professional Certification within 1 year of hire.
  • Bachelor's degree in related area or equivalent experience/training.
  • Experience working with youth and young adults affected by violence or involved with the Justice System.
  • Experience working in a Level 1 Trauma Center Hospital-based Violence Intervention Program.
  • Strong interpersonal and customer service skills.
  • Ability to communicate and resolve issues effectively with a diverse population of patients, staff, and physicians.
  • Excellent analytical and problem-solving skills.
  • Ability to develop solutions and recommend changes and follow through with implementation.
  • Excellent written and verbal communication skills.
  • Proficiency with Windows-based software including Microsoft word, Excel, Outlook.
  • Knowledge of computer systems and software used in functional area.

Qualifications

  • Experience connecting individuals to needed social services in Sacramento County.
  • Experience providing mentorship for youth affected by violence.
  • Personal experience in overcoming violence and violence-related injuries.
  • Knowledge of intervention techniques and trauma-informed practices.
