The Patient Navigator case manages and coordinates aspects of care for treatment of the Complex Care Program by working with the patient and family to provide education and collaboratively establish a plan of care. Serves as a liaison among physicians, nurses, ancillary departments, patients, families, and office staff to ensure effective utilization of resources and coordination of care.
Responsibilities
- Conducts telephonic outreach to patients in need of health care services and educates them on the importance of receiving the needed service.
- Makes sure the delivery of care is on track, identifies issues that may affect the care plan, works with different departments to schedule and complete tests, procedures, and consults.
- Aids patients with durable medical equipment (DME), prescriptions, prior authorizations, and other communications related to the patient/family needs.
- Documents the results of outreach efforts in the patient’s electronic medical record (EMR) and on a tracking spreadsheet.
- Creates and updates the patient’s care plan based on clinical, educational, psychosocial, and cultural needs, and recognizes when to involve other health team members.
- Organizes and prioritizes patient care tasks to make the most of time and resources.
- Performs routine patient care tasks and medication administration in a safe, efficient, and timely manner, and intervenes in emergencies.
- Customizes nursing interventions to meet the patient or family’s needs and adjusts these interventions when necessary.
- Collaborates with registered nurses to reassess and modify the patient’s care plan.
Requirements
- Completion of an approved program per state's requirements.
- 3 years of experience in a related field.
Qualifications
- Licensed Practical Nurse - Arkansas State Board of Nursing (ASBN).
Skills
- Excellent communication skills.
- Ability to manage multiple tasks simultaneously.
- Knowledge of healthcare regulations and policies.

Pay
Most new hires start between $23.00-$28.70 per hour, depending on experience and qualifications.
