Patient Navigator
Job Overview
The Patient Navigator is responsible for coordinating activities surrounding “Patient Centered Care”. “Patient Centered Care” activities include (but are not limited to) identifying current and new patient populations through risk stratification lists and chronic conditions requiring one-on-one support in coordinating care activities, supporting and assisting patients to navigate through all the steps in obtaining the necessary and appropriate care.
Minimum Requirements
- Associate Degree or High school degree with Technical Training, such as Medical Assistant or other healthcare training.
Compensation
