Patient Navigator - Ambulatory Care Network: Emergency - Evenings

About the role

Serve as a patient ally throughout the continuum of care. Coordinate the full breadth of the patient experience by ensuring that the patient is well educated and knowledgeable on navigating the healthcare system and has a comprehensive discharge plan once leaving the ED. Provide non-clinical support to patients by directing them to a primary, specialty doctor, or the service(s) they need. Tap into internal and/or community resources, like neighborhood clinics, to ensure patients get the best clinical care and outcomes.

The schedule for this position is Monday – Friday, 3:00 PM - 11:00 PM.

Responsibilities

  • Navigate patient interactions via phone, on the floor, and across the Emergency Department (ED).
  • Act as one of the first points of contact for patients, providing support and guidance.
  • Educate patients on navigating the healthcare system.
  • Develop and communicate comprehensive discharge plans for patients leaving the ED.
  • Direct patients to primary care, specialty doctors, or necessary services.
  • Connect patients with internal and community resources, such as neighborhood clinics.
  • Manage high-stress situations involving chronically ill populations with complex psycho-social issues.

Requirements

  • High School Diploma or GED.
  • Proficient in Microsoft Office.
  • Ability to manage high-stress environments and work with chronically ill populations.
  • Diverse and culturally sensitive customer service skills.
  • Excellent oral and written communication skills.

Qualifications

Preferred Criteria:

  • Experience working as a patient navigator, preferably in a hospital setting.
  • Bachelor’s Degree from an accredited college or university.
  • Bilingual in English/Spanish.
  • Proficient in Epic.

Pay

Salary Range: $21.35 - $34.15/hour.

Schedule

Evening shift: Monday – Friday, 3:00 PM - 11:00 PM.

All roles require on-site presence, so employees should live within a commutable distance to NYP.
