Job Summary
The Patient Navigator–Community Health Worker supports clients in navigating behavioral health, physical health, and social service systems. Key responsibilities include patient and community engagement, outreach, care coordination, and advocacy. This role works as part of a multidisciplinary team to improve health outcomes, reduce barriers to care, and promote recovery and wellness.

- Able to operate a computer and proficient in Microsoft Office and electronic health record systems
Preferred Qualifications
- Fluent written and verbal skills in English and Spanish
Job Requirements
- High school diploma or equivalent
- 3 years of direct patient care experience
- Community Health Worker Certification or obtain within first three to six months of employment
- Medical Assistant Certification, preferred
- Experience working with diverse populations, including individuals with behavioral health or substance use needs, preferred

$15.35 to $20.59 Non-Exempt
