Patient coordinator

About the role

White Glove Community Care is actively seeking a Patient Coordinator to join our growing team. We are looking for a highly organized, detail-oriented, and compassionate professional who thrives in a fast-paced environment and is passionate about delivering exceptional patient care.

Responsibilities

• Coordinate and manage home care cases and caregiver scheduling
• Serve as the primary point of contact for patients, families, and caregivers
• Handle high-volume calls, follow-ups, and patient inquiries
• Ensure accurate documentation and compliance in HHA Exchange
• Collaborate with nurses, case managers, and clinical teams to support patients' needs
• Monitor attendance, coverage, and missed visits
• Maintain daily, weekly, and real-time updates on case status
• Provide excellent customer service in a fast-paced healthcare environment

Qualifications

• 2+ years experience in home care, healthcare coordination, patient services, or medical office setting preferred
• Strong knowledge of home care operations, scheduling, or case coordination
• Experience with HHA Exchange, EVV systems, or healthcare EMR platforms is a plus
• Excellent communication, multitasking, and problem-solving skills
• Ability to manage high-volume workload and prioritize effectively
• Bilingual (Spanish/English) preferred

Schedule: Mon - Thurs 9a - 5p & Fri 9a - 4p
