Patient Coordinator

Responsibilities
- Manage the clinical support coordination of the patient’s care.
- Primary responsibility for scheduling follow up appointments, surgery coordination and scheduling.
- Support communication across care teams and other Summit departments.
- Communicate information from Physicians and other care providers to patients and other staff, in support of patient care.
- Maintain and complete worklist, in coordination with other identified care team members.
- Accurately enter physician’s orders in to electronic medical record.
- May complete standard (non-complex) patient forms such as FMLA, Short Term Disability, Workers Compensation, and other designated requests.
- Provide patients with information packet and/or documents in accordance with physician protocols.
- Assist with voicemail, email and task lists as needed.
- May be responsible for rooming of patients and initiating visit for physician and other providers/clinicians, as directed.
Requirements
This is a full-time position primarily supporting an Operative Arthritis Physician and Team. Monday - Friday schedule of 7:30 AM to 4:00 PM (subject to change based on clinic and/or team needs). Primary location for this role will be at our Woodlake (Woodbury) Clinic with flexibility to float to other Summit campuses as needed. Other primary sites will likely include our Eagan and Hastings Campuses (subject to change).
Qualifications
- High school diploma or equivalent.
- Experience in a healthcare setting preferred.
- Strong organizational and communication skills.
- Ability to multitask and prioritize effectively.
- Proficiency in Microsoft Office Suite.

Pay
$21.62 to $27.03 per hour.
Schedule
Monday - Friday schedule of 7:30 AM to 4:00 PM (subject to change based on clinic and/or team needs).
