Responsibilities
- Observes, collects, and documents health status data as assigned.
- Completes measurements according to critical elements of policy/procedures, such as temperatures, radial pulses, blood pressures, respirations, weights, intake and output.
- Reports immediately to the RN any observations/health status data that are not within normal limits or have significantly changed.
- Aid patients with activities of daily living (ADL) such as toileting, bathing, dressing, eating, etc., with direction from the Registered Nurse.
- Document according to documentation guidelines.
- Perform personal care/interventions as assigned.
- Assist patients, family members, and visitors with wayfinding on the unit and throughout the facility.
- May assist with patient transfer, movement, and/or rooming.
- Complete admission/discharge/transfer functions per system workflow.
- Distribute and set up water, snacks, and patient trays and assist patients.
- Maintain skin integrity.
- Provide assistance with ambulation, transfer, positioning, and range of motion (ROM) according to activity order and needs of the patient.
- Perform clerical duties and assist with patient registration.
- Accurately and timely process physician orders.
- Thoroughly document relevant patient information (or care) to ensure accurate billing.
- Transport medication from the tube system on the unit to secure locations in accordance with facility standards.
- Clarify, communicate, prioritize, and transcribe written orders per Electronic Medical Record system workflow.
- Order, validate, and store supplies and patient education materials.
- Print Release of Information (ROI) packets for discharge to nursing homes or other facilities.
- Review and explain authorization forms with patients and obtain signatures if applicable.
- Schedule procedures/appointments as needed.
- Contribute to a safe environment for patients, visitors, and employees by reporting environmental risks or equipment malfunctions to supervisor.
- Use equipment following policies, procedures, and manufacturer directions or standards.
- Participate in quality improvement activities to evaluate and improve patient care.
- Handle hazardous waste (identify, segregate, dispose of hazardous waste; ensure container integrity; know emergency response procedures to spills, fires, and evacuation procedures).
- Perform other duties as assigned.
Requirements
- Must meet one of the following requirements: 18 years of age OR 17 years of age with high school diploma or GED.
- Must meet one of the following requirements: Completion of one semester of clinical rotations with an accredited nursing program OR Successful completion of Certified Nursing Assistant (CNA) program OR Completion of the Allina Health Nursing Assistant Academy Program OR NREMT certification upon hire OR Previous direct patient care experience.
- Licenses/Certifications: Must meet at least ONE of the requirements below: Current BLS certification from the American Heart Association Current BLS certification from the American Red Cross Allina in-house BLS training (within 30 days of hire).
Qualifications
- Physical Demands: Medium Work: Lifting weight up to 32 lbs. occasionally (Allina Safe Patient Moving Policy), up to 25 lbs. frequently.
- FTE: 0.75 FTE (60 hours per two-week pay period).
- Shift: 12-hour Day / Night shifts.
- Weekends: Every 3rd weekend.
Pay
Pay Range: $22.49 to $30.51 per hour.
