About the role

A PCS2 contributes to AHF's mission by presenting a friendly, helpful face to patients, people seeking testing, or pharmacy patrons. This position involves greeting patients courteously, scheduling appointments, and assisting with client needs to provide standout service that enhances AHF's reputation and encourages continued medical treatment. The role includes conducting patient intake interviews, financial screenings, and registering clients for Ryan White and AIDS Drug Assistance Program (ADAP) services. The PCS2 ensures appropriate documentation for patient eligibility is filed and entered into the computer database.

Additionally, the PCS2 accurately collects new patient medical history, determines necessary tests/screenings, takes patient vitals, and documents health-related complaints to provide excellent patient-centered care. They communicate with patients between provider visits for condition updates and follow through on tests/procedures ordered by providers to ensure high-quality care.

Responsibilities

  • Greet patients in a courteous and sensitive manner.
  • Schedule appointments and assist with client needs.
  • Conduct patient intake interviews and financial screenings.
  • Register patients for Ryan White and ADAP services.
  • Ensure appropriate documentation for patient eligibility is filed and entered into the database.
  • Collect new patient medical history and determine necessary tests/screenings.
  • Take patient vitals and document health-related complaints.
  • Communicate with patients between provider visits for condition updates.
  • Follow through on tests/procedures ordered by providers.
  • Manage the referral process by processing specialty referrals in a timely and accurate manner.
  • Provide patients with pertinent information for timely appointment scheduling.
  • Assist with appointment scheduling as needed.
  • Obtain specialist consults and scan them into the patient’s EMR.
  • Track and monitor the status and progress of referrals.
  • Notify providers of any lapses in patient referral completion.
  • Maintain contact with patients to improve adherence.
  • Remain flexible and professional in high-stress environments.

Requirements

  • Medical Assistant Diploma or Certification.
  • Valid CPR Certification from an American Heart Association-recognized vendor.
  • National certification is preferred but not required.
