About the role

A PCS2 contributes to AHF's mission by greeting patients, scheduling appointments, assisting with client needs, conducting patient intake interviews, financial screenings, and registering for Ryan White and AIDS Drug Assistance Program (ADAP) services. This role also involves taking patient vitals, documenting health-related complaints, communicating with patients between provider visits, managing the referral process, and tracking the status of referrals.

Responsibilities

  • Greet patients courteously and sensitively
  • Schedule appointments and assist with client needs
  • Offer patient intake interviews, financial screenings, and register for ADAP services
  • Accurately collect new patient medical history
  • Determine what tests/screenings are due during medical visits
  • Take patient vitals and document all patient health-related complaints
  • Communicate with patients between provider visits for condition updates
  • Follow up on test/procedure orders by providers
  • Manage the referral process by processing specialty referrals in a timely and accurate manner
  • Provide all pertinent information to patients for timely appointment scheduling
  • Assist with appointment scheduling as needed
  • Obtain specialist consults and scan into patient’s EMR
  • Track and monitor the status and progress of referrals
  • Notify providers of a patient’s lapse in completing the referral
  • Remain in contact with patients to improve adherence
  • Add consistent value in high-stress environments

Requirements

  • Medical Assistant certification
  • National certification preferred but not required

Qualifications

  • Licenses/ Certifications: Medical Assistant Diploma or Certification & Valid CPR Certification from an American Heart Association-recognized vendor
